The Centers for Disease Control and Prevention recommend getting a flu shot as the first and most important step in protecting yourself during this cold and flu season.

Please note:

• Consent forms will be available prior to the clinic date. They should be filled out in advance and handed to the clinic.
• Please make a copy of the front and back of your insurance card. You will need to give the photocopy to the clinic. 
• You will also need to show photo ID or identification.

The flu shot clinic will be administered by The Cooperman Barnabas Geriatric Health Center.
