The Centers for Disease Control and Prevention recommend getting a flu shot as the first and most important step in protecting yourself this cold and flu season.

Please note:

• Open to all staff and members
• Advanced registration is required. To pre-register, visit: jccmw.us/flu-shot
• Consent forms will be available prior to the clinic date. They should be filled out in advance and handed to the Clinic
• Please make a copy of your photo ID to give to the clinic
• You will also need to show photo ID or identification on the day of immunization

The flu shot clinic will be administered by The Center for Geriatric Health and Disease Management.
